The team at Harborview Medical Center in Seattle, Washington has developed a comprehensive collaborative drug therapy management program. Their experience has led to an in-depth understanding of both, the process of developing such a model, and the challenges of implementation in the actual work environment. This book project would capitalize on this experience and capture the knowledge amassed by the staff in a usable handbook on the process. It would act as both a handbook for collaborative drug therapy management, and as well as offering case studies to highlight the process of creating collaborative services at their practice facility.
